Plans of Safe Care
Improving Family Health with Plans of Safe Care
Plans of Safe Care (POSC) are recognized by policy leaders, health care providers, community members, and families as an important means to coordinate care across systems and improve health for families affected by prenatal substance use.
Pilot Information
Families affected by prenatal substance use often face stigma and fear of child welfare involvement, which can prevent them from seeking treatment and support. Plans of Safe Care (POSC) create a way for trusted organizations and providers to step in early, helping families get the care they need and deserve.
To strengthen this work, a pilot project is underway to implement a data-informed framework for coordinated POSC that can be replicated across Colorado.
The Goal
A strategic, data-driven approach to POSC that supports families through care coordination while adapting to the needs of different communities.
The Focus
- Encourage proactive, voluntary initiation of POSC in community-based settings.
- Strengthen the role of health and human service providers.
- Increase opportunities to begin POSC during pregnancy.
How It's Working
The pilot team partnered with local stakeholders to co-design workflows and implement the POSC framework. This approach was built upon existing community strengths, filled gaps, and was adapted to unique needs.
By learning from this pilot, Colorado can create a shared understanding of how to best deliver Plans of Safe Care—ensuring families have coordinated, compassionate support from pregnancy through the first year postpartum.
